• Description:
• Tenders are invited for Supply of Medical Equipment to Primary Health Centre in Langtang North and South Federal Constituency Interested and competent bidders are requested to collect Bid Documents from the Procurement Department of JUTH upon presentation of evidence of payment of a non-refundable tender fee of Ten Thousand Naira (N10,000.00) only PER LOT. In case of payments made into JUTH-s Treasury Single Account (TSA), evidence of such payments should be taken to the Accounts Department of JUTH for confirmation and issuance of receipt for bid collection. Qualified, competent and interested bidders should have proven evidence of the under-listed documentations that would accompany all tenders: i. Evidence of Certificate of Incorporation issued by Corporate Affairs Commission including the Form CAC1.1 or CAC 2 and 7; ii. Evidence of Company-s Income Tax Clearance Certificate for the last three years (2018, 2019, 2020), expiring on 31st December,2021 with minimum average annual turnover of N10 million for Works; iii. Evidence of Pension Clearance Certificate valid till 31st December, 2021 (applicable where number of staff is at least 15); iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021 (applicable where number of staff is at least 5 or at least N50m turnover); v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021; vi. Evidence of Interim Registration Report (IRR) issued by the Bureau of Public Procurement (BPP) as evidence of registration on the National Database of Federal Contractors and Service Providers expiring 31st December, 2021; vii. A Sworn Affidavit;  That the company is not in receivership, insolvent or bankrupt.  That no Director of the company has been convicted in any court for any criminal offence, including fraud, any financial impropriety or is bankrupt.  Disclosing whether or not any officer of the relevant committees of Jos University Teaching Hospital or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars viii. Current audited accounts for the last three years (2018, 2019, 2020) duly stamped by a registered Auditor; ix. Reference Letter from a reputable commercial bank indicating willingness to provide credit facility to execute the project when needed; x. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of award, Job completion certificates, and clear, colour photographs of completed projects or valuation certificates for ongoing projects; xi. Company profile with a brief Curriculum Vitae of key personnel that would be deployed for the project including copies of their academic and professional qualifications such as COREN, NIQS, NIOB, ARCON; xii. Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria, for Supplies of Medical Equipment; xiii. All documents for submission MUST be transmitted with a covering/forwarding letter on the company-s letterhead paper bearing amongst others, Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact address, Telephone Number (GSM No), and a valid active e-mail. The letter-head paper must bear the names and nationalities of the Directors of the Company at the bottom of the page duly signed by authorized officer of the Company. Additional Info: a. No bidder should bid for more than two (2) Lots; b. The tender documents should be listed and arranged in the order specified in 3.0 above and clearly separated by dividers. c. Certificates may be referred to the respective issuing authorities for verification and authentication; d. Technical and Financial Bids enclosed in one and same single envelope that will expose both the Technical and Financial Bids at the same time during opening shall be disqualified; e. Bid documents shall be submitted in person or courier and JUTH would not be responsible for late couriers; f. All bids submitted shall be made only in English Language; g.
