• Description:
• Tenders are invited for Supply and Installation of Dialysis Equipment Interested Contractors/Service providers are required to submit the following documents: i. Evidence of Certificate of incorporation issued by Corporate Affairs Commission (CAC) including Form CAC 2 and CAC 7 (or CAC 1.1). Business Name is also acceptable for Consultancy; ii. Current Tax Clearance Certificate (or Personal Income Tax Clearance Certificates of all the Partners in case of Business Name) for the last three years (2018, 2019&2020) valid till 31st Dec 2021; iii. Evidence of Pension Clearance Certificate valid till 31st Dec 2021 iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st Dec 2021; v. Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st Dec 2021; vi. Interim Registration Report (IRR) expiring 31st Dec 2021 or valid Certificate issued by Bureau of Public Procurement (BPP); vii. Supply of Equipment: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria. viii. Company-s audited accounts for the past three year 2018, 2019 & 2020); ix. Company profile with the Curriculum Vitae of key experienced personnel to be deployed for the projects including copies of their academic/professional qualification (in case of construction: COREN, QSRBN, ARCON, CORBON etc.) x. Verifiable documentary evidence of at least three (3) similar jobs successfully executed within the last five years including Letters of Award, Valuation/Job Completion Certificates xi. Reference letter from reputable bank and a commitment to provide facility for the execution of the contract, if awarded. xii. A Sworn Affidavit: (a) that none of the Directors of the Company has been convicted of any criminal offence (b) Disclosing whether or not any officer of the relevant Committees of the National Obstetric Fistula Centre, Abakaliki or Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the Company. (c) Confirming that all information presented for the bidding are true and correct. xiii. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm-s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm. Collection and Submission of documents: i. Tender documents are obtainable from the Office of the Secretary Tenders Board Admin Block Upstairs Room 3.09 on presentation of a copy and original receipt of non-refundable tender fee of N10, 000.00 paid into National Obstetric Fistula Centre Abakaliki (Remita Account) for each lot accordingly. ii. Completed technical and financial bids for each lot (three sets of hard copy and 1 soft copy) shall be submitted in two different sealed tamper proof envelopes and labelled “Technical Bid” and “Financial Bid” respectively, and both placed in a third envelope. All the 3nos envelopes should bear the “Project Name and Lot No. at the Top Right Hand Corner of the Envelope and addressed to: Medical Director National Obstetric Fistula Centre Abakaliki, 86 Abakaliki-Enugu Expressway, Abakaliki, Ebonyi State with Company name clearly written at the reverse side of the envelope iii. All documents should be deposited in the tender-s box at the New Admin complex of National Obstetric Fistula Centre Abakaliki not later than 12:00 noon on Monday, 2nd August, 2021. Late submissions shall be returned un-opened. iv. Expression of Interest (EOI) for the Training should be packaged in one envelope and submitted not later than 12:00 noon on Monday, 2nd August, 2021. v. The technical bids and the EOI shall be opened immediately after the deadline for submission at 12:00 noon on Monday, 2nd August, 2021 at the New Admin complex of National Obstetric Fistula Centre Abakaliki, while the financial bids will be kept un-opened. Note: i. Bidders should not bid for more than two (2) Lots; ii. All document submitted will be verified from relevant agencies; iii. Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording (link will be sent to bidders) with the presence of two (2) representatives of private sector professional bodies and Civil Society Organization in the areas of Anti-Corruption/Transparency; iv. Bid documents can also be collected in soft copies and Bidders can submit through Post (Courier Services); v. Goods & Works: Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened; vi. Services: Only shortlisted firms will be invited at a later date for collection of Request for Proposals; vii. The NOFC, Abakaliki is not bound to shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007; viii.
