• Description:
• Expression of Interest are invited for Purchase of Two (2) 400 Ltr Autoclave Machine, Two (2) 50kg Industrial Washing Machine, 50kg Industrial Spinner, 2 Roller Pressing Machine & Industrial Drying Machine. Eligibility; i. Evidence of certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC1.1 or CAC2 and CAC7; Business Name is also acceptable for services; ii. Evidence of Company-s Income Tax Clearance Certificates for the last three (3) years valid till 31st December 2022; iii. Evidence of Pension Clearance Certificate valid till 31st December, 2022 (this requirement is only applicable to bidders whose number of staff is 15 and above). iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2022 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder-s annual turnover is N50m and above); v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2022; vi. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2022 or valid Certificate issued by BPP; vii. Sworn affidavit • disclosing whether or not any officer of the relevant committees of the University of Port Harcourt Teaching Hospital, Port Harcourt Rivers State or Bureau of Public Procurement (BPP) is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars; • that none of the directors have been convicted in any court of law for procurement fraud or financial impropriety; • that the company is not in receivership, insolvent or bankrupt; viii. Reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed; ix. Company-s Audited Accounts for the last three years (2019, 2020, 2021), duly acknowledged by the Federal Inland Revenue Service (FIRS); x. Comprehensive Company-s Profile showing organizational structure with curriculum Vitae of relevant key staff to be deployed for the project, including copies of their academic and professional qualifications; xi. Documentary evidence of at least there (3) similar jobs completed in the last five (5) years with certificate of completion or on-going projects with Interim performances Certificates with work status at least 70% completion, letters of wards and photographs of the projects. xii. Medical & Health equipment: letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospital in Nigeria; xiii. Services: evidence of Firm-s current registration with relevant regulatory professional body(ies); xiv. All documents for submission must be submitted with a Covering/Forwarding letter under the Company-s letter head paper bearing amongst others, the Registration Number (RC) as issued by the CAC, Contact Address, Telephone Number (preferably GSM No) fax and email address. The letter head paper must bear the Names and Nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm. 1. Tender documents are obtainable from the Procurement Department of the Hospital within working hours of 8am -4pm (Mondays - Friday) upon presentation of receipt of payments of non-refundable Tender fee of N10, 000.00 only per LOT. 2. Payment should be made to University of Port Harcourt Teaching Hospital, Port Harcourt through TSA Account REMITA platforms at any reputable Commercial Bank in Nigeria. 3. Interested firms in Services will not pay at this stage until after shortlisting. General Information; i. Submission of EOI/Tender does not mean that University of Port Harcourt Teaching Hospital is under any obligation whatsoever to award your company any contract ii. All bids submitted must be in English language. iii. Bids received after the deadline for submission shall be returned unopened. iv. Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording (invitation link will be sent to bidders to join the bid opening) with the physical presence of two (2) representatives of private sector professional bodies and Civil Society Organization in the areas of Anti-Corruption/Transparency; v. Bids documents can also be collected in soft copies and bidders can submit through Post (Courier Services); vi. Goods & Works: Only pre- Only pre-qualified bidders at technical evaluation will be invited at later date for virtual financial bids opening, while the financial bids of un-successful bidders will be returned un-opened; vii. Services: only shortlisted firms will be invited at a later date for collection of Request for Proposal viii. University of Port Harcourt Teaching Hospital, Port Harcourt reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement ACT 2007.
