Quotation are invited for Provision of Health Insurance for One Year. The quotation should be submitted on or before 5pm, November 28th, 2022 S/N DESCRIPTION NUMBER OF STAFF 1 All staff to be covered under NHIS services Family staff - 29 Single staff - 8 2 Additional childhood immunizations (Meningitis, MMR, Typhoid fever, Chicken Pox, IPV, Rotavirus, Pneumococcal conjugate, Hepatitis A & B) 3 Glasses and eyeglasses allowance of up to N50,000 4 No cost limit on Induction of labor and assisted delivery and C/S Emergency and medically indicated electives 5 Adult immunizations (Hepatitis A & B, Typhoid fever etc). Hospitals to be included on this plan Abuja: DIFF Hospital, Nisa Premiere Hospital, Chivar Clinics & Urology Centre, Cedar Crest Clinics, Zankli Medical Center, Kelina Medical Center, DEDA Hospital. Hospitals to be included on this plan Maiduguri: University of Maiduguri Teaching Hospital (UMTH), Royal Hospital, Alphine Clinic, Borno Medical Centre, Kanem Hospital. · All NHIS available hospital within Nigeria · Include exclusion list in quotations
