Prequalifications are invited for Provision of Consultancy Service for Health, Education and Other Human Development Surveys and Researches Requirements: 1. Cover page/letter on company letterhead - the subject should be indicating the category(ies) of service seeking prequalification for. The letterhead should have the company registered address(s), official, functional emails, telephone numbers, and all points of contact for the company. 2. Evidence of statutory registration document of the company (CAC registration certificate, FORM CO7, and FORM CO2) 3. Tax Clearance Certificate in the last three years (2020, 2019 & 2018) 4. Evidence of Financial Capability (latest Audited Accounts). 5. Bank reference letter duly addressed to FHI360 Prequalification Committee 6. Evidence of registration with Professional bodies, Regulatory Bodies, Manufacturers and/or Certificate of Authorized dealership from original equipment Manufacturer/other companies. 7. Evidence of possession of experience in the line of field/category you are seeking prequalification. The evidence should include copies of completed Job Orders and Proof of deliveries (Delivery Notes or Job Completion Notification/certificate. 8. Three reference letters from three different organizations you have currently or had transaction with in the past attesting to your performance 9. Any other relevant supporting document that could aid your application.
