• Description:
• Tenders are invited for Modernization of Medical Records Equipment and Information System. Interested bidders shall present the following documents in their technical Tender: i. Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including CAC1.1 or CAC2 form7. Business is also acceptable for Services; ii. Evidence of current Pension Clearance Certificate valid till 31st December, 2021. iii. Evidence of Company-s Income Tax Clearance Certificate for the last three (3) years (2018, 2019 and 2020) valid till 31st December, 2021; iv. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021; v. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2021 or valid Certificate issued by the BPP. vi. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021. vii. Verifiable documentary evidence of at least three (3) similar jobs in the last five (5) years including Letter of Awards, Valuation Certificates, Job Completion and photographs of the projects; viii. Reference letter from reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the projects when needed; ix. Company-s Audited Accounts for the last three (3) years: 2018, 2019 and 2020; x. Sworn affidavit that: • firm is not bankrupt; • none of the Directors is an ex-convict or bankrupt; • disclosing whether or not any officer of the relevant committees of Federal Medical Centre, Umuahia or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars; • that the Company would comply to Local Content Policy: xi. Company Profile with Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic/Professional qualification and registration with professional bodies (COREN, QSRBN, ARCON, CORBON etc.); xii. List of Plants/Equipment with proof of Ownership/Lease; xiii. Category C: Evidence of firm-s registration with relevant regulatory professional body(ies); xiv. Lot D12: Only companies included in the National Automotive Design and Development Council-s approved list of local manufacturers of vehicles or their authorized representatives should bid for the vehicles; xv. For Supply of Equipment: Letter of Authorization as representative of the Original Equipment Manufacturers (OEMs); xvi. D11 & D13: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria; xvii. All documents for submission must be transmitted with a covering /forwarding letter under the Company/Firm-s letter Head paper bearing amongst others, the Registration Number(RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone number (preferably GSM No) and e-mail address. The letter head paper must hear the names and Nationalities of the Directors of the company on the bottom of the page, duly signed by the authorized officer of the firm. The bid document can be obtained from the procurement unit, Federal Medical Centre, Umuahia upon an evidence of payment of non-refundable fee of Ten Thousand Naira (N10,000.00) only paid per Lot, into Federal Medical Centre, Umuahia-s Remita Account in any reputable Commercial Bank in Nigeria. Category C: Firms interested in Category C will not pay at this stage, until after shortlisting. General Information: i. Bids must be in English Language and signed by an official authorized by the bidder. ii. Bids must be in Naira Currency iii. Bids submitted after the deadline for submission would be returned un-opened. iv. All costs will be borne by the bidder v. The Federal Medical Centre, Umuahia reserve the right to annul the process at any time without incurring any liability in accordance with section 28 of the Public Procurement Act 2007, vi. Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording (link will be sent to bidders) with the presence of two (2) representatives of private sector professional bodies and Civil Society Organization in the areas of Anti-Corruption/Transparency and shall be made available to bidders upon request, vii. Bid documents can also be collected in soft copies and Bidders can submit through Post (Courier Services) viii. Category A&B: Only Technically qualified bidders will be contracted at a later date for financial bids opening while financial bids of un-successful bidders will be returned un-opened. ix. Category C: Only shortlisted firms will be invited at a later date for collection of Request for Proposals x. This advertisement shall not be construed as a commitment on the part of the Centre or shall it entitle tenderers to make any claims or seek any indemnity from the Centre by virtue of their response to the Advertisement. xi. All CAC, PENCOM, ITF, NSITF and TAX certificates maybe referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue (FIRS), National Pension Commission and other relevant bodies for verification xii. Prospective Contractors should ensure they sign the register provided on submission of bid documents.
