• Expression of Interest are invited for Computers and Printers Repairs (e.g., Preventive, and Corrective Maintenances, Parts Replacements, and Supplies). A company cannot express interest in more than two (2) categories. Hard copies of the EOIs documents should be bound together, guided by the following: (a) must have an operational presence for each category Abuja-FCT (b) sealed in an envelope and marked on the top right side with category reference. Example “IHP/FCT/001 HOTEL SERVICES”, (c) The EOI submission must be addressed and submitted to the State Director of Finance and Administration, USAID Integrated Health Program (IHP), and submitted to the location(s) of interest as listed below: IHP/OFFICE(S) ADDRESSES FOR SUBMISSION Abuja-FCT Office Integrated Health Program (IHP) Nigeria 2nd - floor room 9&10 The Plaza, 23 Ibrahim Tahir Lane, Cadastral Zone B05 Utako, Abuja, Nigeria SECTION D: ELIGIBILITY AND SELECTION CRITERIA The minimum eligibility requirement and selection criteria areas follows: (1) Cover Page (indicating category(s)of interest and related CODE Number(s). (2) Company Profile; registered address(s), official/functional emails, telephone numbers, and point of contact for the company. (3) Evidence of legal registration of the company (CAC registration certificate, FORMC07, and FORM Co2); (4) Latest Tax Clearance Certificate for the past three years; (5) Evidence of Registration with relevant Regulatory bodies (hotel service) (6) Evidence of possession of experience in the line of business you are applying for. Where possible, provide reference letters from at least five (2) reputable organizations, preferably NGOs, attesting to an active or past business relationship and level of performance (the letters or lists must have relevant addresses, contact persons, telephone numbers, and emails) for due diligence confirmation; ADDITIONAL INFORMATION (1) IHP is Value Added Tax (VAT) exempted but deducts withholding tax (WHT) in compliance with relevant Nigeria Tax Laws. (2) For clarification on any aspect of this expression of interest advertisement, please send an email to: FCT.procurements@IHP-Nigeria.com quoting “CALL FOR EXPRESSION OF INTEREST, IHP” as the subject of the email. Kindly quote the reference number for specific details about goods/services/work category(s). (3) As part of the prequalification process, IHP may visit the location(s) of prospecting suppliers to assess their physical presence and capability. (4) Only successful pre-qualified suppliers would be Invited to participate in IHP routine Request for Quotation (RFQ) or Request for Proposals (RFP). SECTION E: DEADLINE FOR RECEIPT OF EXPRESSION DOCUMENT All expression of interest documents must be delivered on or before 5:00pm local time on Friday 30 July, 2021. IHP reserves the right to accept or reject any or all the submissions without any obligation to inform the affected applicants of the ground for acceptance or rejection. The potential subsequent procurement will be guided by the IHP Procurement Policy and Procedures. This expression of interest advertisement is provided at no cost to a prospecting suppliers.
